3. We are a no-kill center. Euthanization shall only occur upon the advice and approval of a licensed veterinarian. Euthanization shall only be performed by a duly licensed veterinarian. A certificate must be sent to the Center by the vet stating the reason for euthanization.
4. Under no circumstances shall an adopted horse be used in any manner deemed unsuitable by the Center. This includes, but is not limited to, racing, gaming, jumping, or any other activity that could endanger the health or even the life of the adopted horse. If during an inspection, the horse appears to be worked beyond its limits, the horse shall be returned immediately to the Center and the adoption is void.
5. If, for any reason you are unable to care for the adopted horse, the Center must be notified and arrangements will be made to pick the horse up and return it to the Center, no refund or exchange will occur.
6. No horse can be sold or traded during the two-year period set according to the by-laws of the Center.
7. All immunizations must be reported annually by a duly licensed veterinarian to the Center. If you are approved to give the stated immunizations, a written statement must be provided to the Center stating the immunizations and date administered.
8. All farrier work must be scheduled and performed on a regular basis per the farrier’s instructions.
9. Deworming for intestinal worms shall be performed at regular six-week intervals.
10. Annual (or as otherwise may be indicated by a duly licensed veterinarian) teeth floating shall be performed on the horse.
11. If the horse is injured or ill, a duly licensed veterinarian must be called immediately and the horse shall be examined and treated as soon as possible at Adoptor’s expense.
12. The Center and/or its representatives, corporate officers, registered agent, or any other individual representing the Center shall not be held responsible for any unknown health problems or conditions which may or may not be present at the time of adoption.
13. The Adoptor represents and maintains that they are an individual over the age of eighteen (18) and are to be legally responsible for the care and maintenance of the above-described horse.

Temporary Placement
The Center hereby authorizes the temporary placement of the above-described horse for a period of fourteen (14) days. This trial period will begin as of this date. At the end of the trial period, the horse will either be approved for adoption and the fee paid, or immediately returned to the Center by the proposed Adoptor. Post-dated check no. _______ in the amount of $______ shall be held by the Center during the trial period.
RELEASE AND HOLD HARMLESS AGREEMENT
I, ________________________, do hereby agree to hold harmless and indemnify the Center from any and all liability and/or ordinary negligence, and further release them, their Board of Directors, individually and/or jointly, their heirs and legatees, executors and administrators remised, and forever discharge from any and all liability, manner of actions, cause and causes of action, ordinary negligence, suits, debts, sums of money, bonds, and claims whatsoever in law or in equity, and particularly, without limiting the generality of the foregoing, all claims and/or causes of actions arising from any accident, loss, or damage to life or property, injury or illness, loss of life, including but not limited to, the undersigned or to any family member or spectator accompanying the undersigned during activities.
The Adoptor also assumes the risks inherent in all horse-related activities, including but not limited to, bodily injury and physical harm to horse, rider, property and spectator.
UNDER THE EQUINE ACTIVITY LIABILITY ACT, EACH PARTICIPANT WHO ENGAGES IN AN EQUINE ACTIVITY EXPRESSLY ASSUMES THE RISKES OF ENGAGING IN AND LEGAL RESPONSIBILITY FOR INJURY, LOSS OR DAMAGE TO PERSON OR PROPERTY RESULTING FROM THE RISK OF EQUINE ACTIVITIES. 745 ILCS 47/1 et seq.
I have read and agree to the above and fully understand it's meaning.
No minor child(ren) shall be allowed to participate in any riding or activities involving the horse listed above unless a separate waiver is included for each minor child(ren) and executed by both parties.
